Condensation was dripping from fan in walk-in cooler near the storage shelf underneath.
Coke machine area cabinets had grease and debris on handles. Also noticed were debris and old spills under coke machine that need attention.
Ceiling tiles need cleaning due to dust and grease build up especially near air vents and in back by ice machine.
Flies were observed in kitchen area.
Microwave observed with dried food debris. CA Person in charge (PIC) advised to clean microwave at least every 24 hours to prevent food debris accumulation.
Aluminum foil observed lining prep table.
Soup in reach-in cooler (RIC) that was cooling from the day before was observed above 41°F. Corrective action (CA): Person in charge (PIC) was advised to ensure items cool with the 2-stage cooling method within 6 hours. Corrected on-site (COS): PIC discarded soup.
Soup was observed in RIC in large pot cooling. CA: PIC advised on proper cooling methods.
Food employees observed without hair restraints. Corrected on-site (COS): When asked if hair restraints were available the employees found hairnets and put them on.
Wet cloth was observed stored in a sanitizer bucket without sanitizer solution. Person in charge (PIC) was advised to only store wet cloths in sanitizing solution.
Small amount of mold observed in bulk ice machine in server area of kitchen. Corrective action (CA): Person in charge (PIC) advised to clean ice machine often enough to prevent mold.
Several containers on shelf above 4-compartment sink observed stacked wet. CA: PIC advised to make sure to air dry fully before stacking. Corrected on-site (COS): PIC stacked containers to allow air flow.
Chlorine sanitizer for dishwasher observed too low and all buckets of quat sanitizer observed too low. Corrective action (CA): Person in charge (PIC) advised to ensure chlorine sanitizer is between 50-100 ppm and that quat sanitizer is at 200 ppm by using test strips regularly. Correted on-site (COS): The bucket of sanitizer for the dishwasher was replaced with a new full container and all quat sanitizer in buckets was replaced.
Several items in reach-in cooler (RIC) #3 such as cooked black beans, cooked lentil beans and cooked corn were observed past held time maximum 7 day date. CA: PIC advised on date marking procedure. COS: PIC discarded items.

Dining tables observed to be sanitized using one bucket of sanitizer and a common cloth. Corrective action (CA): Person in charge (PIC) advised to properly sanitize tables using a spray bottle and disposable towel for each table or 2 buckets of sanitizer with 2 separate towels.
Floors under drink station in drive-thru area and floors under equipment and in vent hood area observed with grime and food debris.
Chlorine sanitizer for dishwasher reading zero ppm on test strips. Quat sanitizer in buckets reading zero ppm. Corrective action (CA): Person in charge (PIC) advised to get dishwasher fixed and until dishwasher is fixed either use 3-compartment sink or wash dishes in dishwasher and sanitize using quat sanitizer in 3-compartment sink. Corrected on-site (COS): Quat bucket was replenished and and PIC advised to use test strips to ensure quat sanitizer is at 200 ppm in concentration.
Hot dogs observed in meat sink in pan of water. Bag of meat observed in meat sink thawing in ambient temperature. CA: PIC advised on correct thawing methods.

Large plastic container of freshly chopped salad was observed cooling in make table with a lid. CA: PIC advised on proper cooling methods. COS: Salad was placed in shallow metal pans and moved into freezer to cool quickly.

Several items were found above 41°F on pizza make table top. They included cheese, chicken, sausage and chicken wings. PIC said items were last checked for temperature at 10:00am. Items were discovered at 3:30pm. CA: PIC advised to ensure that items that are time/temperature control for safety (TCS) foods on make table be held at 41°F or below at all times. COS: PIC discarded items not held at temperature. Temperatures rechecked and 41°F and under.
Employee was observed draining water out of sauce bag in hand sink by pizza make table. Corrective action (CA): Person in charge (PIC) advised that hand sink is for hand washing only and prepping ready to eat foods (RTE) must be done only in the back prep sink. Corrected on-site (COS): Employee was told to go to back prep sink to finish task.

Employee was observed washing food storage containers in the hand wash sink near ice machine. Corrective action (CA): Employee was advised that the hand wash sink was for hand washing only and dishes would need to be taken to be washed in the dish sink.

The employee was noticed using a cloth towel to dry hands and did not wash for 20 seconds. Employee was then asked to was hands properly again. There were no paper towels provided at hand-wash sink. PIC was asked to place paper towels at hand-wash sinks for employee use. Another employee was observed not using a paper towel to turn off faucet once finished with hand washing. The employee was instructed on proper hand washing methods.
Chicken made yesterday at 2:00 P.M. found out of temperature at 44 degrees. It had been cooked, placed in large plastic bin with plastic wrap, and then placed in cooler. PIC discarded the out of temperature chicken.
